Our pricing is simple….
Enrollment Fee
$100 per person
$150 per family
Children, ages 0-17 $30 per month*
Young Adults, ages 18-24 $50 per month
Adults, ages 25-44 $70 per month
Adults, ages 45-64 $85 per month
Adults, ages 65+ $100 per month
Monthly Membership
*All patients 18 years and younger must have one parent or guardian as a member
The Monthly Membership includes your full primary care including your annual physical, all office visits, including sick visits and after-hours phone access for urgent concerns.
Labs and medications are not included in the membership, but are offered at a deep discount.
We do request that You make a one-year commitment to the practice.
This is for your health and ours.
Direct primary care provides you with continuous access to a physician who knows you and can advocate for you within the healthcare system. This typically means less stress and healthcare burden for you and your family, fewer emergency room visits and hospitalizations, lower laboratory and medication costs, but only if you maintain the patient-physician relationship.
As the Direct Primary Care Model is intended for continuous care, we Have the following enrollment termination policies:
We require 30 days written notice prior to membership cancellation
We require a re-enrollment fee, subject to availability, for those patients who cancel or allow their memberships to lapse.
