  • Under the direct primary care model, we do not bill any insurance plans. If you do have health insurance, you can use it for services like labs, imaging and visits with specialists. We recommend that you call your insurance to understand what they cover.  

  • At Polaris Medical, we will take the time to get to know you and your health goals. We prioritize easy access to our services, transparent pricing and the highest quality, whole-person care. Having a primary care physician who knows and cares about you is worth it!         

  • Concierge practices often bill your insurance as well as charge you an annual or monthly fee. We do not bill insurance, so you will never have a co-pay with us. Not participating in insurance allows us to keep our overhead low, which helps us keep our prices low, regardless of your insurance. Our low prices for care also allow us to care for people with little or no insurance.

  • If it’s a true emergency, call 911 or go to the nearest Emergency Room. We are not affiliated with any specific hospital system, but we can get your records from any local hospital and help coordinate your care afterwards.

     If it’s something urgent, but not life threatening, call us first. We may be able to see you right away or help you know what to do next.

    If you need to see a specialist, we can refer you to any hospital or provider just like any other primary care office. We will also help coordinate your needs with that care provider and, as always, be available to help you navigate the healthcare system.

  • Our patients can call us for help with a sudden illness or injury, or if they have forgotten their medications when they are out of town. We can help with virtual visits and are sometimes able to order labs and medications. Occasionally, we may say you need to see someone locally in person for more immediate care.

  • As of January 1, 2026, you may now be able to use your HSA account to may for your membership fees. You can also often use your HSA/FSA to pay for labs and medications through our office. Please check with your HSA/FSA provider or your employer for specific details.

  • Our physicians have decided against prescribing narcotic and benzodiazepine medications over the long term. They are happy to help you through an acute illness or injury with medications at their discretion, but if the condition persists and becomes ‘chronic,’ we will refer you to a specialist who is better equipped to manage your long term, controlled substance needs.

  • Your enrollment fee and your first month of membership will be charged the first month you sign up and your membership is confirmed by the practice.

  • In your emailed invoice, you should see a breakdown of the assessed fees. Often, a lab fee, prescription or advanced procedure fee will be added on as a one-time charge to your monthly membership fee. If you prefer to pay for the extra product or service when it is received, please let us know and we can push that charge through immediately.

  • Yes, the predictability of steady monthly income allows us to keep our prices low and offer a wide variety of free services and discounts.
